Obsessive-Compulsive Disorder across the Shrink Network.

The loop, the medications that change it, the therapy that breaks it.

Reviewed by Shariq Refai, MD, MBA

Obsessive-compulsive disorder is often misunderstood as being about neatness or order. It isn't. It's about a loop. An intrusive thought arrives. The thought creates distress. The distress drives a compulsion (checking, washing, counting, reassurance-seeking, mental review) that briefly relieves the discomfort. The relief teaches the brain that the compulsion was necessary. The loop reinforces itself, sometimes for years.

The good news: OCD has a specific, well-studied, reliably effective treatment. Exposure and response prevention (ERP), combined when needed with an SSRI at a higher-than-typical dose, is what changes the loop. Generic talk therapy and reassurance-giving, although well-intentioned, often make OCD worse.

Each site in the Shrink Network covers OCD from a different angle. Shrinkopedia provides the clinical reference. AnxietyResource provides the OCD Map, the visual cluster page that lays the loop out as one connected picture. AnxietyResearch covers the ERP evidence. PsychiatryRx covers the SSRI dosing. shrinkMD provides telepsychiatry care from clinicians who actually know how to do ERP.

Exposure and response prevention for OCD

The evidence layer. What the research actually says about ERP as the active treatment for OCD and how SSRI dose differs from depression treatment.

SSRIs for OCD: higher doses, longer trials

The medication layer. Why OCD typically needs higher SSRI doses than depression, the longer-than-usual response timeline, and which SSRIs have the strongest evidence.

When the loop is the problem

The pattern layer. ERP is the treatment, but understanding the loop structure, and recognizing it in the moment, is part of what makes ERP work. shrinQ addresses the loop directly.

On the loop

The reflective layer. Essays on OCD as a relationship with thought rather than a content theme, and the slow work of changing how you respond to the loop instead of what you think about.

OCD is one of the conditions where finding clinicians who actually know how to treat it, with ERP, with the right SSRI dose, with patience for the longer treatment timeline, is often the hardest part. The cluster above collects the network's coverage in one place, so the search for the right treatment path is at least starting from a complete picture.
