DSM-5-TR to ICD-11 crosswalk for 65 conditions
The US uses DSM-5-TR diagnoses with ICD-10-CM billing codes. ICD-11 is the World Health Organization's current international classification. This table lines the three up for every condition on Shrinkopedia, links each ICD-11 code to the WHO's own browser, and says in one line where the two manuals disagree.
Where the two manuals part ways
Most conditions carry across cleanly. The differences cluster in a few places, and they're worth knowing before you compare research or records across countries.
Personality disorders changed the most. ICD-11 dropped the named types entirely. It has one diagnosis, personality disorder (6D10), graded mild, moderate or severe, with optional trait domains. That affects 9 rows in this table. Schizotypal disorder moved the other way: ICD-11 files it with schizophrenia, not with personality disorders.
Sleep-wake disorders left the mental disorders chapter in ICD-11 and got their own (chapter 07); 5 rows here are coded there. Substance use splits differently too. DSM-5-TR has one use disorder graded by severity, while ICD-11 separates a harmful pattern of use from dependence.
Some diagnoses exist in only one manual. Burnout, Complex PTSD and Postpartum psychosis have their own ICD-11 code but no DSM-5-TR diagnosis. DMDD (disruptive mood dysregulation disorder)is in DSM-5-TR with no ICD-11 equivalent. For a disorder-by-disorder analysis, see the comparison by First and colleagues in World Psychiatry (2021). How to read the codes
DSM-5-TR doesn't have codes of its own. It lists the ICD-10-CM code that US clinicians bill with, so that's the column shown here. Where a code ends in x, the final characters depend on specifiers such as severity, episode or subtype, and you'll need the manual to pick the exact one. ICD-11 codes link to the WHO browser, where you can see the full definition and the codes nested under it.
The United States still uses ICD-10-CM for billing, while ICD-11 is the WHO's current international standard. This table is for understanding and comparison. It isn't a coding tool, so check the official release before you code a claim.
How we built and check this table
We didn't type these codes from memory. Every ICD-11 code, title and link was pulled from the WHO's official ICD-11 release file, and every ICD-10-CM code was checked against the CDC's official code files, including the release that takes effect each October. A script re-checks every code whenever a new WHO or CDC release comes out, and anything that changes is corrected and logged on corrections and updates. The one-line notes on differences were written from the WHO structure itself and reviewed by a board-certified psychiatrist.
