First-line is the treatment that guidelines recommend a clinician start with, when no specific reason exists to start elsewhere. It's not the same as "the best treatment" or "the only treatment that works." It reflects a weighted balance of evidence strength, effect size, side-effect burden, and cost, applied to the average patient. Individual patients may need something else based on their history.
